Nursing Home Facilities must attest annually that account opening disclosures are given to residents, in accordance to Regulation DD.

• Signed and completed Attestation forms must be returned to NursingHome@AmalgamatedBank.com no later than December 15th of each year.

As a reminder, Nursing Home Facilities must obtain acceptable identification of the residents prior to opening the resident account.

• Failure to provide the IDs at bank audit request can result in closure of the account and suspension of service.
• The ABFast program will be updated to ensure acceptable IDs are obtained and recorded.

As a reminder, Nursing Home Facilities clients must maintain the correct account documentation for residents. Nursing Home Facilities are required do the following for all existing and new residents:

• Ensure residents complete and sign the Agreement and Authorization for Care Facility Residents form (Click here for the form)
OR
• Ensure the Facility completes and signs the Representative Payee's Request to Open Account(s) at Amalgamated Bank For Benefit of Health Care Facility Resident form (Click here for the form)
  o Provide a copy of the Social Security Administration "Retirement, Survivors and Disability Insurance" Representative Payee Assignment Letter
